Transcript request form for current seniors
This form, also available in the counseling center, is required in order for the high school to send your transcript to a college. Using this form, you can also instruct the guidance office to send your college entrance test scores (SAT, Subject Tests, ACT), teacher recommendations, and other materials.

Mid Year Report request form for current seniors
Use this form to request that the guidance office send a mid-year transcript or mid-year report form to one or more colleges. This form is also available in the guidance office beginning in January.

Complete and return this form (mail, email or fax) to have a copy of your high school transcript sent to a college or employer.
